Ensuring a trouble-free discharge for patient and professional
For some patients, the move from hospital back to home can raise concerns. The Charter Healthcare Hospital to Home service offers a smooth transition from hospital to home care for patients requiring continence or stoma products.
Following assessment by hospital staff or a Coloplast Clinical Nurse Advisors, all prescription supplies will be delivered free of charge to the patient's home for use on discharge. Our Post-a-Script service then ensures a reliable, free of charge prescription delivery service right to the patient's door.
This means you will only need to send the patient home with the quantity of product stated in your discharge contract, which is usually a one week supply. It is also extremely cost-effective as the patient receives the goods they specifically need, when they need them. And, of course, our Customer Care service team are on hand to offer help and support if you have any problems.
Ward Procedure - Prior to discharge, you should always:
- Ensure your patient is familiar with the product they are being discharged home with, e.g. how to change the Leg Bag, how they should link to the Night Bag and how they are emptied.
- Please provide them with the appropriate supporting booklet / literature.
- Explain the Charter Healthcare service and provide them with the Charter patient guide.
